If you wish to enrol your child or children into our school please come and pick up an application form from our administration area.
Alternatively, you can find the information in our forms and downloads page. The Application for Enrolment document and the Enrolment Form provide all the necessary information required for the initial enrolment application process. You can either return the completed forms to our office or email them to: queenspark.ps@education.wa.edu.au.
